If the one-hour time block does not work for you, you may come at a different time that evening. We are trying to keep waiting time to a minimum by splitting up team testing times.

Download the SWAY Medical app BEFORE you come to testing. (Free In the app store)
Make sure you bring your phone and your phone is charged. (SWAY testing will use your phone)
There will be two portions for testing this year, one portion will be on your phone, and the other will be individual with a tester.
Testing takes about 45min once you begin, so please plan for at least an hour.

Registration: Grades (7-12) for this 2024-2025 academic year.
Teams: Any team using the weight room must have students register online and pay the fee.
Fee: $55.00 one-time per student fee for the summer of 2024. NO free or reduced fee available. All fees for this program will offset the costs associated with staffing, equipment and supervising the facility.
Sessions Begin: Monday, June 3, 2024 and conclude Thursday, August 1, 2024 (Monday-Thursday) (NO sessions July 1 thru July 6)
**The workout time slot signup link will be emailed to the student (each student will have to sign up for 1 workout time slot).
All time slots are in the morning. We will not let students sign up for a time slot UNTIL they have registered and paid online.
This summer program is ONLY open to NEXT school year's (2024-25) grades 7 - 12
First day of Fall practice/tryouts is Monday, August 12 for ALL FALL SPORTS - you will NEED to be registered online in order to practice/tryout.
The new Minnesota Law states that all students will receive free lunches this school year (2023-24), if a student would like to qualify for a scholarship to participate in Athletics or Activities at the high school level, parents need to fill out the form in THE LINK BELOW. This needs to be completed every school year starting in July.

SPORTS PHYSICALS - ATTENTION 2023-24 ATHLETES:
You NEED to take the MSHSL physical form with you. Physical form can be found on this website (bottom of page - Tab FORMS).
Directions for finding physical date in Skyward:
Sign in to your skyward family access and look under the "Report Card & Portfolio". This will be listed with all of the testing results and report cards.
You will need to have a copy of your physical (page 1 and 2) in the Athletic/Activities Office BEFORE you can register online. The system will not let you register without this.
Once you get your physical done, please email it to Dakota Hiller at dahiller@rochesterschools.org. AS a PDF file OR WORD document (attachment) NOT in the body of the email.
Your updated physical needs to be on file in the Athletic/Activities Office at your high school BEFORE you can register online. Physicals need to be current for the ENTIRE Season in order to participate. They cannot expire before June 15th, 2024.

This will be by scheduled appointments only and starting on May 13 for the prescheduling form.
Athletes or parents need to follow this link or QR code to complete a pre-scheduling form which should go-live on May 13. The QR code is also on the attached flyer.
Once the pre-scheduling form is submitted, they will be called in 2-3 business days to schedule the sports physical in Sports Medicine. Calendars for scheduling will not be available until May 13. If webform is submitted prior to that, call backs will not be until May 13 or after in the order received

There are three timeframes parents may choose to be called to schedule the appointment which include: 7 a.m. to 12 p.m., or 12 p.m. to 4 p.m., or 4 to 7 p.m. If we are unable to reach them via phone at their requested time, we will send an email or Patient Online Services message.
Cost is $20 by cash or check only, payable at check-in on July 27.

Free Heart Health and Spleen Study
Mayo Clinic Research is offering an electrocardiogram (ECG), an echocardiogram, and two ultrasounds of the spleen for all sports physical participants at no charge.
If the parents/athletes say "yes" on the webform, research coordinators will contact the family prior to July 26 to answer questions regarding the study as well as obtain consent prior to the day of the physicals. We anticipate this station of the PPE will add about 30-45 minutes at the end of the PPE exam.
